One Call Medical This was Raytel. It works the same way. One Source will call us to schedule. We should be given a confirmation number. Have patient sign Radiology Release Form. Offices can call 1-800-872-2875 and ask for scheduling department. When calling One Call tell them we need a "Single Case Agreement" for a group health claim.

Oxford Health Plans - via Valley Preferred Provider ID: A3479868 LVDI contracts all Oxford insurances through United Health Care or Valley Preferred so you may see a UH or VP symbol on the card but it is still an Oxford plan. As long as the word OXFORD is on the card, follow the guidelines for Oxford. See O on grid. For precertification of CT Scans call Care Core at 1-877-773-2884. MUST inform offices that precertifications do go through CareCore. Please provide offices with correct prompts. Oxford secondary to any other insurance MUST be precertified ...2ND to MEDICARE does not need precert! Must also give offices the CPT codes as they are contrast related authorizations. These authorizations usually begin with "A"

It is NOT necessary to have a DATE OF SERVICE to obtain a precertification through ALL imaging companies. Offices must use 1230 S. Cedar Crest Blvd as our address when precertifying. At this time offices should be using our NPI number 1912956954 when certing as well. Most Medicare REPLACEMENT plans require precertification. These plans replace Medicare. When a patient states they have Medicare, ask if they have a secondary insurance plan. Some plans may require a pre-cert. Check the grid. LVDI is in network for ALL plans through Valley Preferred. Please check grid or the card for a Valley Preferred symbol.
